Clinic Registration FAQ

Q: I'm trying to register, but I don't see a link to do that. Where is it?

A: The registration button is at the top of the larimerflu.org "home" page. It is in a box that says "Register for H1N1 (swine) Flu Vaccination." If you don't see this registration link at the top of the home page, please refresh the page using the "refresh" button. If the registration box still doesn't appear, please wait about 15 minutes, refresh the page and try again. If, after doing these things, you still don't see a registration button, contact us.

Q: Where is my registration confirmation number?

A: On your registration form, at the top left is a barcode. Your confirmation number is after the barcode.

Q: I am not able to open and print my registration form. What should I do?

A: About five minutes after you registered, please check the e-mail account of the e-mail address you provided. You should receive a confirmation e-mail with your registration form. You will need the latest version of Adobe Reader to open your registration form. This can be downloaded from get.adobe.com/reader.

Q: I did not receive a registration confirmation e-mail. What should I do?

A: If you printed your registration form or remember your confirmation number, that is confirmation of your appointment. Please remember to bring your printed registration form or confirmation number with you to the clinic.

If you do not have a printed registration form and do not remember your confirmation number, please use the appropriate method of determining your registration confirmation number:

  • If you supplied an e-mail address when you registered, find your confirmation number by submitting that e-mail address in the "Cancel Registration" section at the bottom of the registration page. All appointments for that e-mail address will be listed with confirmation numbers. Find the appointment you are looking for, and please remember to bring that confirmation number with you to the clinic. DO NOT SELECT THE APPOINTMENT AND CLICK ON THE CANCEL CHECKED REGISTRATIONS BUTTON UNLESS YOU WANT TO CANCEL THAT APPOINTMENT.
  • If you did not supply an e-mail address when you registered, using the "Contact Us" section of www.larimerflu.org, fill out your name and e-mail address, and submit the following information in the "Questions and Comments" box. We will get back with you as soon as we can.
    • That you are requesting a registration confirmation number
    • The name of who was registered
    • What date and time slot were registered for
    • Your phone number

Q: I need to cancel an appointment. How do I do that?

A: If you supplied an e-mail address when you registered, you can cancel your appointment online.

If you did not supply an e-mail address when you registered, using the "Contact Us" section of www.larimerflu.org, fill out your name and e-mail address, and submit the following information in the "Questions and Comments" box. This will cancel your appointment.

  • Your registration confirmation number
  • The name of who was registered
  • What date and time slot were registered for
  • What type of vaccine (shot or nasal spray/mist) was registered for

Q: I need to reschedule an appointment for a different time the SAME DAY. How do I do that?

A: If you supplied an e-mail address when you registered, cancel your appointment online, and then re-register. If the time slot you want is not listed, it is not available.

If you did not supply an e-mail address when you registered, using the "Contact Us" section of www.larimerflu.org, fill out your name and e-mail address, and submit the following information in the "Questions and Comments" box. We will get back with you as soon as we can.

  • Your registration confirmation number
  • The name of who was registered
  • What date and time slot were registered for AND what time slot you would like to be rescheduled to
  • What type of vaccine (shot or nasal spray/mist) was registered for
  • Your phone number

Q: I need to reschedule an appointment for a DIFFERENT DAY. How do I do that?

A: If you supplied an e-mail address when you registered, cancel your appointment online, and once registration opens for the date you prefer, please register again.

If you did not supply an e-mail address when you registered, using the "Contact Us" section of www.larimerflu.org, fill out your name and e-mail address, and submit the following information in the "Questions and Comments" box. Once registration opens for the date you prefer, please register again.

  • Your registration confirmation number
  • The name of who was registered
  • What date and time slot were registered for
  • What type of vaccine (shot or nasal spray/mist) was registered for

Q: I registered for the wrong type of vaccine and need to switch to the other. How do I do that?

A: If you are unsure if you registered for the correct type of vaccine (shot or nasal spray/mist), please come to the clinic during your scheduled date and time slot. Screeners at the clinic will help you make a determination.

If you are sure you need to switch and supplied an e-mail address when you registered, cancel your appointment online, and then re-register.

If you did not supply an e-mail address when you registered, using the "Contact Us" section of www.larimerflu.org, fill out your name and e-mail address, and submit the following information in the "Questions and Comments" box. We will get back with you as soon as we can.
